Vision Coverage

For an annual eye exam, there is no copay in-network, with the plan covering 100% after the copay. Out-of-network reimbursement is up to $45. Lenses, including single, lined bifocal, lined trifocal, and lined lenticular, have a $0 copay in-network, with 100% coverage after the copay.

Dental Coverage

The dental plan provides in-network benefits, including no annual deductible for some options and a $50 individual or $150 family deductible for others. Both options offer a $1,500 annual benefit maximum per person. Preventive care is covered at 100% with no deductible, while basic care is covered at either 100% or 80% after the deductible.

PTO (Vacation, Sick, Personal Time off)
